Fees
- Initial Applications: Non-refundable $700.00 registration fee + $25.00 BCI background check processing fee
- Renewal Applications: Non-refundable $700.00 registration fee
Required Documents
- A copy of the agreement given to the client with the disclosures required in Utah Code §13-52
- Surety in the amount applicable according to Utah Code § 13-52-302(10) on the approved Division form.
- For initial applications and renewals adding new principals only:
- BY MAIL ONLY: An FD-258 fingerprint card from each principal with all required fields filled in correctly (see Required Fields on Fingerprint Card Form for more information) DO NOT email or fax in fingerprint cards.
- A Background Check Consent Form for each principal submitting fingerprints to the Division
- For renewing applicants only:
- A Request to Terminate RapBack Consent Form for any principals(s) who are no longer key participants of the seller
