Apply for a Massage Therapist License

If you are currently licensed as a Massage Therapist in a state, territory, or district of the United States you may qualify for licensure by endorsement. Refer to Endorsement to Utah: Massage Therapists for additional information.

The following requirements must be met:

Fingerprint cards. Refer to Fingerprinting to visit our Fingerprint Information Page for details
Request MBLEX or the NCBTMB examination results be sent directly to DOPL
Education:
- Verification of completion of massage therapy education OR
- Verification of completion of approved massage apprenticeship including apprentice logs, and passing score the Utah massage therapy Law & Rule exam
