Apply for an Occupational Therapist License

If you are currently licensed as an Occupational Therapist or Occupational Therapist Assistant in a state, territory, or district of the United States you may qualify for licensure by endorsement. Refer to Endorsement to Utah: Occupational Therapist / Occupational Therapy Assistant for additional information.

The following requirements must be met:

Have requested Official transcripts from the school documenting graduation from an approved program with a Bachelor's or graduate degree. Have official transcripts sent via secure email from your school's registrar's office to [email protected]
NBCOT certificate
I certify that I have completed a minimum of 24 weeks of supervised fieldwork as an Occupational Therapist.
Fingerprint cards: If you are utilizing the DOPL fingerprinting services, please submit your licensing application prior to making your appointment for fingerprinting. Visit our Fingerprint Information Page for details.

The number one reason for license denial is the lack of fingerprints. To ensure your application does meet this end, we recommend you follow this easy tips:
- If you are getting your fingerprints done utilizing DOPL, you must first submit your application then schedule your fingerprinting appt with DOPL at doplfingerprints.youcanbook.me. This ensures your fingerprint can be attached to your application and no denial letter will be sent.
- If you are using an outside agency to get fingerprints done, we must receive the original fingerprint cards with your application, or shortly thereafter. The typical processing time frame for a complete application is 21 days. If we have not received your fingerprints prior to processing your application, it is likely you will receive a denial letter. Fingerprint cards can be mailed to: Division of Professional Licensing: PO Box 146741, SLC UT 84114 or brought in person to the Heber M Wells building at 160 East 300 South, Salt Lake City, Utah.
