Renewal Requirements
Osteopathic Physician & Surgeon Compact Renewal Requirements
If you obtained your Utah professional license, ending in -1234, through the Interstate Medical Licensure Compact (IMLC), the following renewal requirements must be met for all Osteopathic Physician and Surgeon licensees.
Licensees with a Controlled Substance (CS) license:
If you hold a Utah Controlled Substance (CS) license, you can renew both licenses during step 3. Ensure you meet the requirements outlined in Utah Code 58-37-6.5 and Administrative Rule R156-37-402.
- $193 renewal fee
- Complete 40 hours of CE as indicated in R156-68-304.
- A minimum of 34 hours must be AOA or ACCME Category 1
- Complete at least ONE of the following online suicide prevention trainings
- 3.5 Controlled Substance CE credit hours approved by the Division
- $78 renewal fee
Renewal Instructions:
- Go to imlcc.com, click on the renewal link.
- Follow the IMLC Renewal Requirements, including the fee(s).
- Once the Division receives the required IMLC renewal document, you will receive further instructions via email on how to complete your Utah renewal.
- Once the renewal for your Utah IMLC-Osteopathic Physician & Surgeon license has been approved, you will recive a copy of your updated license via the email address on file.
NOTE: All the steps must be completed in their entirety prior to your current expiration date to finalize your Utah license renewal. Submitting your IMLC renewal documents through IMLCย will not automatically renew your Utah License. Your license may lapse if you do not complete/submit all of the above requirements for your IMLC renewal prior to your expiration date.

Additional Information
- DOPL will send a renewal notice to the email address on file 90 days prior to the expiration date. Utah Code 58-1-301.7 requires you to notify DOPL of any change to your address or email address within 10 business days. You can update your information at any time by visiting My License One.
- Continuing to practice a profession after a license has expired is considered unlawful conduct and a criminal offense, per Utah Code 58-1-501(1)(a) and 58-1-502. DOPL may take disciplinary action for any period of time worked without a license, or for failing to meet renewal requirements, even after the license has been renewed or reinstated.
