Transcripts are considered “official” when they are sent directly from the school to DOPL or sealed in an envelope bearing the school’s stamp/seal on the envelope flap.

If you are currently licensed in a state, territory, or district of the United States you may qualify for licensure by endorsement. Refer to: Endorsement to Utah: Anesthesiologist Assistant map for additional information.

The following requirements must be met:

Graduate from an Anesthesiologist Assistant educational program accredited by Commission on Accreditation of Allied Health Education for the Anesthesiologist Assistant.
Current certification with the National Commission for the Certification of Anesthesiologist Assistants (NCCAA).
Obtain and submit your National Practitioner Data Bank (NPDB) report.
