Transcripts are considered “official” when they are sent directly from the school to DOPL or sealed in an envelope bearing the school’s stamp/seal on the envelope flap.
If you are currently licensed in a state, territory, or district of the United States you may qualify for licensure by endorsement. Refer to: Endorsement to Utah: Anesthesiologist Assistant map for additional information.
The following requirements must be met:
- Graduate from an Anesthesiologist Assistant educational program accredited by Commission on Accreditation of Allied Health Education for the Anesthesiologist Assistant.
- Current certification with the National Commission for the Certification of Anesthesiologist Assistants (NCCAA).
- Obtain and submit your National Practitioner Data Bank (NPDB) report.
