- National Exam — administered through the ADA
- Dentists: Part I & II OR the INDBE are released electronically. If you did not request to have the scores released to Utah at the time the exam see the ADA website at https://www.ada.org/en
- Dental Hygienist: The exam is released electronically. If you did not request to have the scores released to Utah at the time the exam was taken see the ADA website at https://www.ada.org/en
- ADEX (formally WREB, CDCA, CITA and NERB), the exam released electronically. If you did not request to have the scores released to Utah at the time the exam was taken see the ADEX website at https://adextesting.org/
- SRTA: The exam is mailed to the division. To request scores see the SRTA website at https://srta.org/
- CRDTS The exam released electronically. If you did not request to have the scores released to Utah at the time the exam was taken see the CDCA website at https://www.crdts.org/
- Local Anesthesia Permit allows for local anesthesia and nitrous oxide
- Minimal Sedation Permit allows for minimal sedation with nitrous oxide and enteral medications
- Moderate Sedation Permit allows for moderate sedation through the use of IV medications
- Deep & General Sedation Permit allows for deep sedation and general anesthesia

No. It is considered unprofessional conduct to engage in practice as a dentist or dental hygienist without prominently displaying a copy of the current Utah license.
A licensee is not required to inform the division if they choose to allow the license to expire.
Inactive status is available if the license is active and in good standing. You must apply for this status by application and pay the applicable processing fees. It is important to note that inactive licenses, the license holder must continue to pay renewal fees.
You must have a current license in good standing and have been practicing for five consecutive years immediately preceding the submission of the Utah application.
