Renewal Requirements

Dental license renewals are due on May 31st of even years and require the following:
  • $73 renewal fee
  • Current CPR/BLS
  • Continuing education hours as outlined in Utah Administrative Rule R156-69-304a

Minimal Sedation Permit

  • 30 hours of continuing education which includes:
    • At least 2 hours specific to the adminstration of enteral anesthesia-pharmacology and minimal sedation.

Moderate Sedation Permit

  • 30 hours of continuing education, which includes:
    • At least 4 didactic hours specific to moderate sedation and anesthesia
    • Successful completion of at least 10 sedation cases

    Deep Sedation & General Anesthesia

    • 30 hours of continuing education, which includes:
      • At least 8 didactic hours specific to general anesthesia
      • Successful completion of at least 30 sedation cases.

      Controlled Substance (CS) License Renewal Requirements

      Controlled Substance licenses renew at the same time as the main license and require the following:
      • $78 renewal fee
      • 2 hours of controlled substance prescribing specific continuing education as outlined in Utah Code 58-37-6.5

      Renewal Instructions

      This process is most effective when done on a computer rather than a mobile device.
      1. Log in to My License One using your UtahID. If you do not have a UtahID, you will be directed to create an account.
      2. Once logged in, scroll down to the "Existing License/Registration Holders" section.
      3. If your license isn't listed there, click the "Don't see your license . . ." link.
      4. Search for your license using the details in your renewal notice (emailed to you 90 days before your expiration date).
      5. Once your license is listed, click on the three dots under the "Actions" column to start the renewal process.

      Additional Information

      • A receipt will be sent to the email address on file up to 2 business days after the payment is made. NOTE: fees are non-refundable.
      • A copy of the license will be sent to the email address on file 1-3 business days after approval.
      • DOPL will send a renewal notice to the email address on file 90 days prior to the expiration date. Utah Code 58-1-301.7 requires you to notify DOPL of any change to your address or email address within 10 business days. You can update your information at any time by visiting My License One.
      • Continuing to practice a profession after a license has expired is considered unlawful conduct and a criminal offense, per Utah Code 58-1-501(1)(a) and 58-1-502. DOPL may take disciplinary action for any period of time worked without a license, or for failing to meet renewal requirements, even after the license has been renewed or reinstated.
