ANNOUNCEMENT

The Division of Professional Licensing has discontinued its in-house fingerprinting services. If you are applying for a license that requires a fingerprint based background check, follow the "Submitting Fingerprint Cards" instructions below.

Many DOPL licenses require applicants to submit fingerprints. These fingerprints are used by the Division for a fingerprint search through the files of the Utah Bureau of Criminal Identification (BCI) and the Federal Bureau of Investigation (FBI).

Submitting Fingerprint Cards

Utah applicants must have their fingerprints electronically submitted through an authorized vendor by providing them with the fingerprint authorization form. Follow the steps below.

1. Submit Your Application: Applicants must complete and submit all other requirements
for initial licensure.

2. Authorization Form & Vendor List: You will receive a fingerprint authorization form along with a list of approved vendors by email approximately 2-3 business days after applying.

3. Contact Vendor: You will then need to contact the vendor to complete your fingerprinting requirement.

  • You will be required to show the vendor the authorization form that was emailed to you in step 2.
  • You will be required to show a valid, government-issued photo ID (driver's license, state-issued ID, passport, etc.) to the vendor.
  • You may have to provide contact and demographic information to the vendor.
  • You will have to pay the vendor's fee for this service.

Please note, your application is incomplete until you complete this requirement. To avoid delays, complete this requirement as soon as possible. You only need to send in fingerprints one time per application, unless you receive a fingerprint rejection email.

Out-Of-State Applicants

To obtain fingerprints, you must go through a third-party vendor, such as your local police station or a private fingerprinting service. They must be able to print out two (2) copies of your fingerprints onto the blue federal form (FD-258) cards.

Cards must have complete demographic information (all aliases, age, hair/eye color, place of birth, etc.) and be signed by both the applicant and the individual who took the prints. Prints must be free from smudges and be rolled completely from nail bed to nail bed, and from the tip of the finger to below the first knuckle. Incomplete or low-quality cards will be rejected, and may delay the issuance of your license.

Fingerprint cards can be mailed to the Division of Professional Licensing, State of Utah Department of Commerce, P.O. Box 146741, Salt Lake City, UT 84114-6741.

Privacy Act Statement

Prior to submitting fingerprints, you must read and acknowledge the Privacy Act Statement, fbi.gov/services/cjis/compact-council/privacy-act-statement . You will find a Criminal History Disclosure Statement in your applicant, that includes a link to this form and an affidavit acknowledging you have read the information and consent to the background check process. This affidavit is a required part of your application, whether you obtain your fingerprints at the DOPL offices or submit physical cards from a third party vendor.

Review of Your Criminal Record

FBI

If you wish to review or challenge the accuracy of the information in your FBI record, you should contact the agency that contributed the information in question.

BCI

If you wish to review or challenge the accuracy of the information in your BCI record, you must complete the required “Record Challenge Form”, available at bci.utah.gov/criminalrecords/criminal-records-forms/ , and submit it directly to BCI.


Agency Review

Agency review of a licensing decision based on your criminal record may be obtained by filing a written request for agency review with the Executive Director of the Department of Commerce within thirty (30) days after notification of the decision. Any such request must comply with the requirements of Utah Code § 63G-4-301 and Utah Admin. Code R151-4-902.
