Apply for a Substance Use Disorder Counselor License (Utah Applicant)

The following requirements must be met:

Official transcript documenting completion of prerequisite courses from a regionally accredited institution of higher learning that covers the following subjects:
- Human Growth & Development Across the Lifespan.
- General Psychology
Official transcript documenting completion of a substance use disorder program from a regionally accredited institution of higher learning that includes 200 hours of substance use disorder education, including at least 2 hours of training in suicide prevention, and completion of a 200-hour supervised practicum.
Completion of a 2-hour continuing education course in suicide prevention.

NOTE: Official transcripts must be sent directly to the Division from your University.
