Apply for an Audiologist License

Military Resources: If you are a military member or military spouse, please review our Military Resources page before applying.
If you are currently licensed as an Audiologist in another state, territory, or district of the United States you may qualify for licensure by endorsement. Refer to Endorsement to Utah: Audiologist for additional information on requirements and how to apply.

Provide documentation for one of the following:

  • Official transcripts verifying a Degree in Audiology, along with one of the following:
    • Current ASHA Certification
    • Current ABA Certification
    • Evidence of a passing PRAXIS exam score and the conclusion of a Clinical Fellowship or Externship
    • Documentation of the current State Board of Education credential.

OR

  • Documentation of a valid credential from the State Board of Education
