Apply for a Speech-Language Pathologist License

If you are currently licensed as a Speech Language Pathologist in another state, territory, or district of the United States you may qualify for licensure by endorsement. Refer to Endorsement to Utah: Speech-Language Pathologist for additional information on requirements and how to apply.

Provide documentation for one of the following:

  • Provide documentation for one of the following:
    • Official transcripts for master's degree in Speech Language Pathology; and one of the following:
      • Current ASHA Certification; OR
      • Documentation of passing PRAXIS exam and completion of a Clinical Fellowship/Externship.

OR

  • Documentation of the current State Board of Education credential
